Add Attachment to Share project PDF forms

Hi, 

Trying to add attachments on PDF forms which is Share Project - PDF Forms - Share | ServiceNow Developers

tried by adding header and footer images on PDF Forms by capturing into an update set using (Manually Add a Record to an Update Set - ServiceNow Guru). But after few days attachments missing.

Can anyone help?
