Add hyperlink in change management page

Is it possible to add multiple hyperlinks to the Change Management form page? If yes, how to do it?
As per the requirement, I need to add to the respective place.

Sure, just add the URL fields to the change and then modify the Form Layout so that they get displayed above the Process Flow formatter.

Hi Ron,
There is no Test URL field available. Do I have to create a custom field for this?

Yes, In my example I created two custom fields on the form - Test URL and Test URL2 in order to demonstrate the functionality. I was doing this with the assumption that you wanted to save the hyperlink with the change record. However, if you want the same hyperlink to show on ALL change records, you should use the method suggested by @Ian Mildon. With his method, the link(s) are not saved on the Change record and only appear on the form.

Another option that might be viewed as a little "neater" is to add an Annotation (Section Separator) to your form and selecting the HTML button, then popping a HREF formatted URL in the Annotation Text box.
And you can add multiples of these, with each just giving you a clickable link rather than the full URL
