Adding attachments to templates

Does anyone know of a way to add an attachment to a template so when the template is used the attachment is also added to the change, incident etc?

Can't be done with OOTB scripts. You can create your own business rule or modify existing one to implement this concept.

Sorry, I was unable to find OOTB BR/Script that is updating the values in change.

Can you tell me how are you using this template? Is it through record producer or through an "Apply template" UI action.
