Adding Custom Table Fields to Ticket Conversation Widget.

Hi All,

Looking for some help in adding new fields from a custom table to ticket conversation widget so that it can display each of this fields activity as a card as it usually does when a record is created or when work notes/attachments is added. 
My scenario is:

I have a button “Add Activity” when clicked, a popup window appears(as shown in screenshot attached) when I fill those fields and save, these fields should be populated as cards. Each card with each activity.

 

Would anybody be able to help with this?
