Additional approval in the Change Management flow

Hi all, 

I'm trying to create an additional(group) approval in the Assess phase of Change Management flow. 

This is the first time I'm trying to create something using the flow, any help is appreciated.

Steps that I took .

a. Replicated this OOB flow - Change - Normal - Assess

b. Added additional 'Ask for Approval ' action

I'm getting internal error when trying to test this change. I haven't deactivated the OOB flow yet.
