Additional Comment and activities field are not visible on incident form

Additional Comment and activities field are not visible on incident form for few integration related incidents. Even if in form layout it is added and user have admin role. Can anyone help me with this?

This is the back-end form? Agent Workspace? Either way, go to Configure > Form Layout and confirm that the "Additional comments" and "Activities (filtered)" fields are present. They are likely in the Notes section.

 

If they are present, I would look at UI Policies on your Incident table. It's possible that someone configured a UI Policy that hides the fields.
