Additional comments field on Incident form

Hi,
On incident form, there is are two fields under Notes tab- Work Notes and Additional Comments (Customer Visible)
I want to know where these fields are stored/ configured. Because when I search in incident table I found comments as a field with label Additional Comments, but I am doubtful why it is showing Additional Comments (Customer Visible) on form.
One more question I have that from where we can put the placeholder in text fields.

This label is defined in the on load client script named: Modify Comments Label
