AI Search Result Action

Hello, We need to create Incident ticket from result given by AI Search. In AI Search, we have configured Categories table. Now, when user search for any issue, its related category would appear and on clicking category, Incident form should open with pre-filled category and service which is mentioned in search result.

I did setup Result Action but its not working. Please let me know any clue on this.

Thanks for solution but i am using Vancouver version, here its different. See images below where i am stuck

with this i am landing on below page but category is not auto filled,

Not able to pass variable value from url and query in image1 to auto populate these values in image2

still not working. Not getting category field auto filled.

Can you pls post how have you configured the Action - Test?
