Approval action in Flow designer is not adding the approver to RITM

I have created an Action for General Manager approval and its returning the SYS ID of the approver. Now I have added this action to the flow designer, but when I am submitting the catalogue request the general manager approval is not getting attached. Refer attached Snapshots.

You are only showing the flow. Does the execution of the flow show you that you indeed get the managers sys_id as output from your action? And also: are you creating the approval with that sys_id as approver?

Thanks Mark for your response. What changes I need to made to ensure that flow is working correctly end to end ?

Check on your flow executions to see what you are getting. Do you get the manager's sys_id? Are you creating the approval record?
