Approval activity in workflow being skipped and approvals are not being created

Hi!
I'd appreciate some help here.
There's a catalog item workflow that have a manager approval activity when creating a RITM. But there are some cases, that I don't know why, RITM are created without any approval request and when I take a look at the workflow, approval user activity was skipped. Does anyone know why this happens? There's no much information about why approval was skipped and didn’t even created approval request at sysapproval_approver table. I'll share some prints.

@Onoderav Are you doing the Manager approval based on “Requested For” or any other referenced variable?
If so, does that user record hold manager details or is it left blank? If blank, it might skip the approval.

Hi.
Yes, it's based on 'requested for' and the manager was inactive when the RITM was opened. In this case, approval may be skipped?
There's another thing I didn't understand:
After the manager approval, there is a manual approval that was skipped too. Why it didn't wait for the manual approval to be rejected or approved?
I've tested in development instance, and even when manager is inactive it's creating the approval.
