Sounds like you have more than one workflow acting on the request. Try using the Show Workflow related links on the RITM and REQ and see if you can spot any workflows that you don't expect being attached.

Hi ,
I think issue with workflow.Please check any business rule is causing to trigger workflow multiple times , check in system logs and workflow contextes as well.
