Assessment questions are not visible.

Hi All,

I have enable the Change Management - Risk Assessment plugin, I have created Assessment on table change_request as below.

I have not given any condition and Decision Matrix.Then I have created a Category as below.

then created a Assessment Metric with questions as below

Am generating this assessment via UI Action and assigning to a person. When person open the assessment to take assessment, questions are not visible.

Can someone suggest what will be the reason.

I think i ran into this issue before and the fix was to add at least one condition. We added Active is true.

Thanks for the response, I tried the same, now it is thorwing an error to fill the mandatory fields in Decision Matrix tab, can you please suggest what I need to enter there. Below is the screen shot.

I don't have anything like that on my form. Was looking at original screen shots and don't see where that is coming from.

I had this same problem. You have to make sure you set a condition on the Assessment Category not the Risk Assessment.
