Assessment related link , not available on form

I created a Assessment Metric Type , Assessment Category , Assessment Metrics on custom scoped table and Published the assessment and expect to see a Assessment related link. But it doesn't appear.
UI Action isn't created on the table .I also made sure the conditions on assessment category AND Assessment Metric Type are same after referring a post from community.

- Check if the UI Action is created and associated with the table. Ensure that the UI Action is configured correctly and has the appropriate conditions to display the Assessment related link.
- Make sure the UI Action is active and visible to the appropriate user roles.
- Confirm that the UI Action is added to the form or list layout where you expect to see the Assessment related link.
Check Assessment Configuration:
- Ensure that the Assessment Metric Type, Assessment Category, and Assessment Metrics are properly configured and associated with the custom scoped table.
- Verify that the assessment is published and active.
- Double-check the conditions on the Assessment Category and Assessment Metric Type to ensure they match the criteria of the records you expect to see the Assessment related link for.
Verify Security Settings:
- Check the security settings and roles associated with the custom scoped table, Assessment Metric Type, Assessment Category, and Assessment Metrics. Ensure that the appropriate roles have access to view and interact with the assessments.
- Review the user roles assigned to your user account and confirm that you have the necessary permissions to see the Assessment related link.
Clear Cache:

Tried all this , no luck.
Does the UI Action as a related link gets Auto created on Custom tables too ?
