Attachment is not getting updated in REQ/RITM

I have created a request and then I attached a document , I am able to see that document in the REQ item 

but once the req is created when I add another attachment like this

its not getting updated in the REQ/RITM 

why ?

Is it added to the attachment table?

Yes, I can see those attachments in sys_attachment table.

so you are adding attachment to REQ and are expecting it to be seen on RITM?

If yes then this is not going to happen out of the box.

what's your actual business requirement here?
