auto populating field values in dialog box from Form

For an incident task, 
1.when an UI action is clicked it should open a dialog box with all the fields and in incident task form.
2.The fields in dialog box should show already saved information from respective fields.

you can send the field values of form as parameter and set those in UI page/dialog box fields
