Can I create a custom form and assign it to a workflow?

Here is my requirement:
I want to create a custom workflow on the Change table that asks the requestor a series of questions or options, like a checkbox where they request accounts for example in our environments.
- For example. Under the "Change New" there is a workflow that says "Request accounts"
- You are then directed to a "custom" form, different from the normal change form, where there are a list of checkboxes that list systems where the user needs accounts, like "Jenkins", "AWS Console", "Azure Console", etc...
- The requestor fills it out like any other change and the request approvals like any other.
- And the workflow goes thru whatever is in the workflow
Is this possible? Seems like if there is a way to create a custom form and then "attach" it to a workflow would sound simple, but maybe it is not, not sure.
Still learning some of this but not necessarily a rookie...I guess a custom app could be created, but I want to leverage the change table somehow in this scenario...
Any help is appreciated!
