Catalog Item templates on the Service Portal to populate forms

Hello
I'm quite baffled that I can't find any information on using templates to populate forms on the Service Portal.
This is a must-have for our CSM clients who will be populating forms multiple times with very little differences in data.
Has anyone found a way to use templates to populate forms in the Service Portal? Or created their own solution to do this?

Unfortunately this is not a Portal solution. I may end up building something similar to this I was just hoping something OOTB existed or someone had refined a similar process for the portal.
