Catalog item variables are not displaying in the Self - Service --> Raise a Request

I modified the existing user account catalog form by adding 2 new fields in the form. It is displayed in the ServiceNow portal but not in the Self - Service --> Raise a Request. Please find the attached screenshot for reference and help me in fixing. 

 

Under u_task_categories I am able to find this, Please help me

may be some catalog client script or catalog UI policy is hiding them on native view?

  • have you verified existing client scripts?

The first two don't seem like they are the same record producer, one is called "user accounts" and the other "request (internal)"

is the User Accounts(S11 referring screenshot) & Request( Internal) (S12 by referring  screenshot)are same record producer?.
