Change group approval rejected

Hi,

On the change form, a user (Mike) requests a group approval. In the group there is only one person (Anna). Anna approves the request, and in the activity log it successfully says "Anna approved the task." The problem is that the next message on the activity log states that the group approval has been rejected? Even though it says it's been rejected, the change still moves to the next step -> authorize.

Why is does it say group approval has been rejected? 

I have checked the Change Decisions, and none of them have approval action set to reject. 

In the change form itself it also says the group request has been approved, but in the activity log it says otherwise. Any other idea what could be the cause of this?

I probably have more questions than help . . . .

- Mike added a group approval outside of the Approval Policies? If yes, the Flow doesn't know about any manually added approval requests, which is a problem. If the state changes, it should cancel all pending approval requests. If all the Approval Polices are completed, the change could move to approved w/o the manually added approvals being approved/rejected.

- How did Anna approve? On the approval form? If yes, did the Group approval update to approve or did she approve the group approval?

- Anna should be approving the Approval request and the Flow should be updating the Group Approval to Approve.

Hi @Michael Fry1 , thanks for replying, apologies for the late reply on my end.

- Anna approved via the "my approvals" module. The change notes say Anna has approved the task, but the next message says the Group approval has been rejected by Anna. The state then changes to Authorize. 

It's just the message that says "rejected", the overall change flow continues like normal. I don't get why the flow isn't updating the group approval message in the notes to Approve.
