Change Request Form Fields for Azure Cloud Services

Hi,

I am Developing a Change Request Form for Azure Cloud Services, and since i am waiting for our ServiceNow installation Is there any documentation on (or someone who has prior knowledge of):

  1. How ServiceNow helps with Change Requests (Especially for Azure Cloud Services)
  2. Which Fields are available for CR Form after ServiceNow is installed and operational and has discovered all available services in Azure
  3. Can we add Custom Fields
  4. Is there a form available that has all fields populated along with pull down selections for multiple options?

I have an Existing form, which i want to conform to ServiceNow so when it would be available, there would be minimal disruption and redesign required.

 

I have worked on the attached MindMap for the form (initial draft stage)
