Change request is stuck in assess state.

Hello,

We have change request that is stuck in assess state even after approval. One thing noticed is 'approval' field of this change still says requested. This issue is not reproducible but keeps occurring once in a while.

@Kartik Choumal Yes, actually it was one of custom BR that was causing an issue
