Change Request Summary page in Employee Center

We have a requirement to change the request summary page while submitting a request from Employee Center.
From this:

Can someone please provide guidance on how to achieve this?

The 1st screen shot is Standard ticket configuration. May i know the source of 2nd one? As per my understanding it will be good customization , as you need to create a new page and link with on submit button.
