Change success score rate is not showing in change model form

Can some tell why success score rate is not showing , I checked jobs indicators ,breakdowns etc or I'm in missing something here?

Hi Atul ,
Thanks for response, I have checked all above-mentioned points and repaired the plugin as well but still I'm not getting success score% ?
Is there any other way I can check on that let me know.I even checked the jobs related to change success score.

I tried this at my end and it worked, just make sure you have atleast 1 change for 1 model only then it work.
