Change task closed with out close code and notes filled

Hi all,
Currently we have noticed this odd behaviour such as the user was able to close the change task with out adding the close code and notes.
Not all his change tasks are in this manner, as except 3 such tasks all other tasks are closed properly. Even when I checked with that user he explained his process of closing a change task and he always added the close code and notes.
Not able to figure how those tasks are closed in that manner?
or is there any way the data could have been erased? (nothing on the audit history)

Hi @Rahul Raja Sami ,

If issue is not reproduceable then there might be chance that some one closed it using script or from list view of incident.

If UI policy or client script written to make those fields mandatory then if user can close the ticket without filling close notes and closed code from list view.

Even though Close Code and Close notes can be edited but state cannot be edited from List view. The list has different users and multiple tasks are there as such.

Hi @Rahul Raja Sami,

we can verify the condition OOTB UI policy 'Mandatory - Close code and Close Notes if Closed', it is responsible to mandate the close notes for change task table.
