Checkmark on "reviews not completed" is missing for certification task

Hi all,

 

I have created a data certification through the CMDB workspace data manager where checkmark is missing. the certification policy is configured as below:

But as you see below, I miss the checkmark between the pencil and the display name - what could be the reason for this?
