Choices of my Assessment questions not being shown properly

Hi,

I have created an assessment and 5 choices for each of the questions in that.

I have created a Risk Assessment and clicked on "Migrate to Assessment" Related Link. After doing this, my view is as the below snapshot.

When I clicked on "Take Survey" button in the above snapshot, it opened my assessment form which is as follows.

But, when I navigated to "My Assessments and Surveys" under Self Service, I found my assessment form as below. The options are being shown as 1,2,3,4 and 5, but not my choices.

Why is it happening so? I want the survey to be shown as in the 2nd snapshot. Please help.

I have solved this issue by going to Survey --> Questions. Under these, I went to my questions and updated the Dislpay field with my choices which were empty till now. Because the Display value was empty, I was not able to see choices.
