Comments posted to the RITM should appear on the SCTASK and vice-versa

I've created 2 BRs to implement the requirement that the additional comments posted to the RITM form should appear on the SCTASK form and also, additional comments posted to the SCTASK form should appear on the RITM form. The requirement is satisfied and the comments do sync on both the forms without repetition also. The problem is that the additional comments are also visible separately above the Additional comments dialogue box along with being visible under the Activities section on the SCTASK form as seen in the attached screenshot. This is happening only in the SCTASK form not the RITM form. 
Why is this happening and how to solve this issue i.e, remove the comments visible above Additional comments dialogue box and make it only visible under the Activities section.

Can you quickly cross check by right click on form Configure->Form Design/Form Layout and see is there any column added which showcase this data If you find any unknown column remove it and check it again.

It worked. The form design contained additional comments field as well as request.additional comments field. I removed the later one and the issue was resolved. Thank you!
