Confused on how to setup Standard Change. Need some guidance.

I've been asked by management to develop our Standard Change catalog but I'm a bit confused by how this process is supposed to take place. According to what I have seen these are preapproved change templates that do not require approval.
So after choosing "Standard" I then fill out the below template form
Then submit and it creates a STDCHG request which then gets approved by the managers.
After the approval I then get this error.
If I go to the Standard Change Catalog I can see the two I made
If I choose it, it opens up a normal change request with some of the fields copied over.
I was under the impression that after creating these proposals and they were approved that there would be no approval needed. Here is the workflow for the standard change proposal.
Can someone explain how this is supposed to work or point to a good resource or guide on how to set this up? Most of the Youtube videos I've seen have been pretty brief.

Standard changes are basically catalog items. If you go to the module Maintain Items, you will find your standard changes in there with your other catalog items. On that catalog item table, there is a business rule - Message if all flows empty - that checks if Flow, Workflow, or execution plan are all empty and if true, shows that message. Normally when you create a standard change, Execution plan is set to DEFAULT.
Maybe you can check the table - sc_cat_item_delivery_plan - and see if that execution plan still exists?

I'm not able to add an execution plan, it stays grayed out.
The dynamic default value is "Get Default Execution Plan"
After the approval of the template is where im getting the error. The template is still created though. Ideas?
