create a new module

I need to create a new module which asks for a new value from user and updates it in another table where the user wants to change/update a particular value.

for this i need to create a module and a form..can anyoone help with this.

Thanks for the update. Can you please personalize the form layout->Check for the Field "Create Mobile" and save the form? Screenshot for reference.

I was able to create a custom table without selecting any create module checkbox and went to the application menu of the particular application and added a new module and linked this table in the new module creation form.

I am using Istanbul version for my instance.

I created a module within the particular application menu as i do always..but was looking after the create module checkbox that was mentioned by Pradeep as to see how exactly that is different from the current procedure of mine..
