Create "Add" button. When clicked on Add button, data should be pushed in table.

1. Create Catalog item "Access Request" and Create variables as shown in the SS i.e. Category,Short Description, Incorrect Serial Number,Correct Serial Number.

2. Create Add button.

3. When clicked on Add button, data should be pushed in table.

You can try the same with the Record Producer, Record producers are used to pushing the data into target tables directly by mapping the variables to target fields.

But for adding a button I have to use widget in catalog item.

Could you help me for same?

You can use MRVS to fill information

Now creating/pushing data into another table can be done via workflow run script
