Create "Add" button. When clicked on Add button, data should be pushed in table.

1. Create Catalog item "Access Request" and Create variables as shown in the SS i.e. Category,Short Description, Incorrect Serial Number,Correct Serial Number.

why not use OOB catalog and MRVS for it

then use workflow run script

I would suggest to keep it OOB and don't go with custom widget or button

Anyhow if you are planning to use catalog item then you can use MRVS variable
