Create Request in an incident form needs to redirect to portal

HI Community,
I have a requirement, in incident form we have "create request" on clicking that it takes me to this and on clicking that it is shows nothing its empty. on clicking create request it need to redirect to service portal how can i achieve this?

Please check if any category is mapped to this catalog.
If no category is mapped, it will appear blank.
To verify:
Go to Maintain Catalogs → Open the relevant Service Catalog → In the Related Lists, check the Categories section.
