Creating a case and incident when record is submitted

When a record is created and submitted in record producer, a new incident and case has to be created. How do we do this?

I don't get your requirement - is it a case or is it an incident you would like to have created? Case and incident is two different applications in ServiceNow.

Ok, so basic when you create your record producer, you need to select the table for the specific case you would like to create. E.g. if your case is placed at sn_cumsterservice_case, then it should be as follow:
