This is the catalog item with three variables,

once the request is submitted, there should be a notification sent to a particular email, which has the variable and its values as an excel sheet attachment.

the data in the attachment will look like

Requested For	User 1
What issue are you facing?	Not receiving mails
Are you continuously facing this issue?	Yes
