Flow designer catalog RITM showing request approved stage showing

I have created one flow and adding stages waiting approval but RITM  that is showing request approved stage after showing waiting approval stage show i am confusing whenever show the work notes for field changes

Share the flow you created, there might you selected wrong stage/state. 
Need to review the flow first. Check the stages you selected for every step in the flow designer.
