Flow Designer - randomly not generating approvals

Hello,
i have a service request to add users to ServiceNow Groups.
the first part of the flow asks for the requested for manager's approval.
I'd say 80% of the time, there is no issue and the approval for the manager is generated.
however, the other times, the approval is not generated and the ritms get stuck.
when I look at the flow context, it says "complete", but skipped.
I have confirmed that for all of these instances, the person does in fact have a manager the manager is active, the manager is getting other approvals, etc.
(so the action in the flow says complete, but when I look at the flow context it says approval state skiipped)
It seems very random. Can anyone maybe help me troubleshoot? I would really appreciate it. Thank you!

Could you check if there are other approval records for the said group? If there are, then what is their state? If you are testing, make sure that you delete all the old approval records.

It's actually an individual (manager) approval, not a group.

Yes, I understand that. What I meant is that assuming User A has to be added to Group B is the scenario where the approval is skipped. Are there other similar approval records?
If had a case where User A's manager has other approvals related to Group B with state cancelled and then the approval requests were skipped. I deleted the records and they worked normally again. It happened to me due to multiple approval records created while development/testing.

Hi, thank you for clarifying. I understand what you're saying. I checked, and that does not appear to be the case. I was not able to find canceled approval records that fit the scenario. Thanks again for respsonding.
