Catalog Item has 2 Approvals in flow when 1st Approved 2nd Approva generates then 1st Approval reset

For Catalog Item Configured 2 Approvals in flow when 1st Approved 2nd generated then1st Approval resets to Requested Any idea Why?

 

it happens only in Prod and not on Dev and Test Environment

We have turned off the Approval Engine for RITM and it resolved the Issue.

Use Flow Logic "Wait for a duration of time" for about 2 minutes in between the 2 Ask for Approvals.
