How did you set a workflow? Last I knew, incident doesn't have a workflow (or I might be wrong).

You can build a workflow on any table. We built one for follow-up on Incident.

At this point, I'd build it in a Flow using in Parallel where the trigger is the follow-up date changes. The first part of the parallel does your actions and the second part of the parallel is the date changes, end the flow. You need to build it should they change the date, you don't want the flow running on the old date. Hope that helps!
