Form values changes are not getting saved

I am facing a weird sort of issue. Whenever we are trying to save the form changes on few records, it is not getting saved. But the even weirder part is the changes are being reflected on activity history without the corresponding values being saved on form.
Has anyone faced such a issue?
The form appears to be getting saved when we hit 'update' button and redirection too works fine. I am not receiving any error neither do I have any script that could possible abort action.

Hi Kalai,
Have you tried checking all Client Scripts and UI Policies, global and non-global for the form? It sounds like the issue is client side.
If the values are reflecting on the activity history, it tells that the values you entered on the form to update the record was successful. You may also want to check the UI Action as it may have been customized as well.

Activity being update reflects that the updation was successful and hence Client scripts and UI policies cannot impact in any way possible per my understanding. The UI action is update, which is OOB as well.

There aren't any real steps that I can put on. Open any task based tickets, change , incident .
Change any field on the form and click update.
And yes, I have already the BR's related to task table.
PS: We have on-premise instance
