FSM Survey / Questionnaire not editable and reflecting as completed on the Mobile Agent App

I am currently trying to create a survey / questionnaire that would be available to Field Service Engineers on the Mobile Agent App. However, there seems to be an issue with this. I followed the instructions on the ServiceNow docs on how to create this questionnaire in Field Service Management. But when I access the survey on the Mobile Agent App, the survey reflects as completed instead of pending and is not editable at all from the Mobile Agent App. 

However, I am able to access this survey in the backend / classic environment with no issues.

 

Can someone please assist me?

 

I also see that there is a known error article and I even reproduced the steps :https://support.servicenow.com/kb?id=kb_article_view&sysparm_article=KB0965723 but the questionnaire is still not working on the Mobile Agent App.
