Getting error for Add Attachments to form(sp) in ATF

Hello Guys,

Currently I am working in Utah version, I am creating an atf for service portal where i am navigating to Service center and trying to submit a request which should contain an attachment. For that i have user "Add Attachments to the Form (SP). When execute the atf its failing with error "Invalid table". Can any one help me on this please. Please see attached screen shot
