Having a requirement on HR Referral Program Catalog Item

I need to create a HR Referral Catalog item, 

and in that i have to create Multiple variables, but one variable is there, that i'm not sure how to create that,

ie,

Variable Name:: Referring Employee's Signature, for creating this variable, which variable type i need to take?
Please find the below Snapshot for that.

Can somebody please help me on this.
