hide the Discuss and Follow buttons on the Change Request form when state is Closed.

When the change state changes to Closed, both the Follow and Discuss buttons should be hidden from the change form. These actions should not be available once the change is closed.

you can't hide Follow & Discuss buttons based on Condition.

It will be either shown/hidden directly
