How can i add a comment on a service portal widget ?

I'm trying to add a comment and make it mandatory if the approval is rejected.

I think i need to pass from the widget editor so if it is can you help me please with an example ?

Thank you for the reply, i think the link is more about the mandatory part on my question.

In first step i need to add a comment when i click on reject button.
