I'm not sure I understand what you are asking. Are you asking how to add Comments and Worknotes to an extended table's form? Or something else?

I am buiding my own application, i want to have this notes layout in my incident form, how can i add the notes layout in the picture into my incident form?

These are the journal entries, which you can add in your table.

You can get it, you need to add the field on table and bring on form vianform layout.
