You would have to get with the account rep on the project you are on. This is a paid service and only ServiceNow personal can run the HealthScans.

The high-level HealthScan Scorecard is available free. Additional optimization services, such as a more detailed review of your instance via a Configuration Review are available for a fee and can be ordered through the ServiceNow Store. For customers who are preparing to upgrade,its complimentary.

Yes You can login with you Hi Portal account on hi.service-now.com and request for a health scan report else contact your account rep on the project you are on.

Actually, the HealthScan Scorecard is produced after a health scan is ran. It's not free its part of the Health scan cost.
