I built the Application in Studio.
I have created a customer portal index page:
And am trying to link it to a form page. I have created the page but don't know how to add the form fields onto the page.
I am trying to link it to something like this.
Is there a particular widget I should be using?

Good Afternoon,
I'm unsure if the 'application' is any different than a record producer, but that's what the form looks like to me that you want above.
So you'd want to first build out the record producer with applicable settings, table/dictionary entries etc. then when clicking "New" you'd want to right click in the gray header bar and choose Form Design. From there, you'd be able to select the fields you've previously set up and build out your form.
Then in the service portal > Designer > Relevant Page, you'd choose something like "Icon Link" from the widget and then point it to another new page with the catalog item widget on it and then when the user click your icon link on the main page, they'll be taken to your other page with the form laid out as you did in form design.
I may be misunderstanding you, but that's the general idea.
