How do I add phone icon on a field on Service Operation Workspace?

Hello All,

I want to add phone icon on Business phone field on Interaction & Incident form on Service Operation Workspace. 

I am attaching a screenshot of Workspace. Same configuration I want to build for SOW.

The way it's build on Workspace is Workspace > Actions & Components > Field Decorators. I tried to configure the same way for Sow but didn't get success.

I have attached the screenshot of steps I have configured for phone icon in SOW.  Not sure what needs to change here or missing some other configuration too!
