I have a requirment. When RITM is rejected RITM State (sc_req_item) and Requested state (sc_request) both should get update. However only one record is getting updated (attached image for reference). Only Step 6 gets executed. How to correct this and make both 6 and 7 to run?

Hi @vidhya_mouli,
According to your screenshot step 6 is not executed, but cancelled.
I guess you have logic which closes a ritm based on a rejected approval. This cancels the flow running flow.

So how do I ensure that both the records gets updated in the same flow? Is there a way to do this?

You will need to find what closes the ritm, and determine if you indeed want to stop it closing that way. (Because this will most probably affect other ritms as well)
There is no way you can do this from the flow, as long as it gets cancelled by something outside the flow.
