How do you add HTML text to the top of a Questionnaire for a Vendor Risk Assessment?

I have created a questionnaire and added it to a Vendor Risk Assessment which will be sent out. At the top of this questionnaire I would like to post a rating scale so that the user knows how to answer each question properly. Any suggestions on how to go about this with HTML text?

Note that I got this to work pretty easily on a Survey using the Introduction field. However when I add this same field to the Questionnaire, it does not show up from the Assessment Portal view.

I've had the same issue right now. It seems as though the Assessment portal will only pull in the fields that are available on the vendor risk view of the assessment form. Survey view has the introduction field, but unfortunately I do not think the Vendor Risk Assessment has that option.
